Camper Application

If you know a child living with HIV/AIDS we would like to invite them to apply to our summer camp program. We aim to welcome back returning campers who are an active part of our community, as well as introduce new ones to the magic of camp!

Camp AmeriKids is a residential camp which serves children ages 7-15 who are either infected with or affected by HIV/AIDS. Camp is offered at no cost to the families.

The dates for Summer 2009 are:

Camp Session 1: Sunday, July 26 – Sunday, August 2
Camp Session 2: Friday, August 7 – Friday, August 14

Regular Camper Application Psychosocial History Form Information Release Form Also due on this date, a copy of the camper’s insurance card	Camper Medical Application Individual Orders Form Meningitis Information Letter and Vaccination Form HIPAA Form Also due on this date, a copy of the camper’s immunization record
