Camper Application

If you know a child living with HIV/AIDS we would like to invite them to apply to our summer camp program. We aim to welcome back returning campers who are an active part of our community, as well as introduce new ones to the magic of camp!

Camp AmeriKids is a residential camp which serves children ages 7–15 who are either infected with or affected by HIV/AIDS. Camp is offered at no cost to the families.

Sending in your application does not guarantee your child a spot with Camp AmeriKids. We wish we could accept everyone who applies, but space is limited. It is best to send in your completed application as early as possible.

  • Psychosocial History
  • Information Release Form
  • Individual Education Plan (IEP) if in Special Ed
  • Copy of Report Card and Teacher Recommendation (if possible)
  • Camper Medical Application
  • Individual Orders Form
  • Meningitis Information Letter and Vaccination Form
  • HIPAA Form
  • Copy of the Camper’s Insurance Card
  • Copy of Camper’s Immunization Record
